Disclosure Statement

As indicated in the Landy’s Care LLC Service Agreement, Rules and Procedures, and NRS 449.0021, Landy’s Care LLC provides non-medical and in-home personal care assistance for persons with disabilities and the elderly who requires assisted with the Activities of Daily Living (ADLs). It is NOT within the scope of Landy’s Care LLC licenses to manage the medical and health conditions of its clients. Landy’s Care LLC provides care within the home of the elderly or persons with disabilities through its employees and/or by contractual agreement with other persons. Landy’s Care LLC and employees will assist with daily living, including, without limitation:

1. The elimination wastes from the body;

2. Dressing and undressing;

3. Bathing;

4. Grooming;

5. The preparation and eating of meals;

6. Laundry;

7. Shopping;

8. Cleaning;

9. Transportation; and

10. Any other minor needs related to the maintenance of personal hygiene.

This term does not include:

1. An independent contractor who provides non-medical services specified by subsection 1 without the assistance of employees;

2. An organized group of persons composed of the family or friends of a person needing personal care services that employs or contracts with persons to provide services specified by subsections 1 for the person if:

• The organization of the group of persons is set forth in a written document that is made available for review by the Health Division upon request; and

• The personal care services are provided to only one person or one family who resides in the same residence; or

• An intermediary service organization

•  As used in this section, “intermediary service organization” has the meaning ascribed to it in NRS 427.0291
